With the easy Evalato drag and drop builder in Settings > Registration > Registration form, you pick from various field types and place them in the designated area to tailor the registration form exactly the way you want it.
Note that by default, the registration form features a Name and Email field which cannot be removed since the system uses them to identify every registration.
Apart from those, every field you add to your registration form is customizable and has the option to be marked as Required. This means the person making the registration has to fill in the requested piece of information, or they can’t proceed with the registration process. You can also add a description to most fields.
Note that you can also add the same form field more than once, e.g. if you want your registration form to include a work phone number and a mobile phone number, if you want multiple questions, etc.
Main fields
In addition to the email and name fields, the form builder lets you add more main fields to help you identify registrations, namely Phone number, Address, Job title, and Company / Organization.
Custom fields
These include a secondary email (e.g., if you want the option to have some emails delivered to a nominee), a free-form Text field, an URL field, a Country field that will add a drop-down list of all countries to your registration form, a Question field that you can customize with predefined answers, and a choice of checkboxes, radio buttons, or drop-down menu style, and finally, a Waiver field that will add an acceptance checkbox with customizable text. You can use this field to include Terms & Conditions, acknowledgment, or a disclaimer field. Note that you can also choose whether to have waiver text expanded by default for registrants to view or have them open it for viewing with a click.
Pro tip: You can use the Waiver field for marketing purposes such as subscribing registrants to your newsletter, email and SMS alerts, etc.
Design elements
These include a section break with a Label to visually enhance your form and a text block for further instructions and information.
Deleting a field
To delete a field simply click the X on the right of the field you wish to remove. The form builder will then ask you to confirm the deletion.
Note that you can check how your registration form looks to your applicants by clicking on the program’s URL found on the program Dashboard.
IMPORTANT: If you delete a field of a registration form that already has been used and has submitted entries, you will lose the information and this is not reversible.
