Structured multi-input form field explained

The structured multi-input field lets applicants add multiple items in a structured way — each item consists of a text input and a dropdown selection. It's useful when you need entrants to provide a list of items that each requires both a name and a classification, such as crediting the companies involved in a project and specifying their type, listing the team members behind a production and their individual roles, crediting the contributors to a media piece, or listing referees for a scholarship application.
Field settings
- Label — the name of the field as it appears on the application form, for example, "Company credits" or "Production team."
- Required — toggle on if entrants must fill in this field to submit their application.
- Description — optional text displayed below the field label to guide entrants on what to enter.
- Tooltip — optional additional guidance displayed as a tooltip icon next to the field label.
- Max items — the maximum number of rows the entrant can add. For example, setting this to 5 means entrants can credit up to five companies, or list up to five team members.
- Name label — the prompt for the text input field. This tells entrants what to type in the free-text field, for example, "Organization name" or "Full name."
- Type label — the prompt for the dropdown field. This tells the entrant what the dropdown is for, for example, "Type" or "Role."
- Type options — the values that populate the dropdown. Add one option per row. For company credits, this might be Agency, Brand, Venue, or Event supplier. For a production team, it might be Director, Choreographer, Set designer, or Composer. For media credits, it might be Photographer, Editor, Reporter, or Producer. For scholarship references, it might be Academic supervisor, Employer, or Mentor.
How it looks for entrants
Entrants see a table with two columns — one for the name and one for the type — and an Add row button that lets them add more items up to the maximum you have set. For each row, they type a value in the name field and select an option from the type dropdown.
Data export
When you export your program data, all items in a structured multi-input field appear within a single cell, with each item on a separate line in the format Type: Name — for example, "Agency: Blue Events" or "Academic supervisor: Dr. Jane Smith."
